What is Transition Planning?
Transition planning is a set of coordinated activities, outlined in the Individualized Education Plan (IEP), within the school year that a student becomes 15 years of age.
The activities help students choose and plan for what they would like to do beyond high school. This planning includes:
- Partnering to develop a road map leading to post high school service planning
- Utilizing IEP as an ongoing vehicle that reflects and guides the transition
- Accessing the areas of employment, housing, benefits, medical and leisure services, as well as any other areas believed to be important for the student to become a successful member of the community
- Preparing the student for the future pathways available after graduation

Jawonio Summer Education Program
Jawonio’s Summer Education Program provides an educational and recreational extended school year program for children ages 5-21 with disabilities. This 6-week summer day program provides academic and recreational programming by certified special education teachers and related-services according to each child’s IEP (Individualized Education Plan).
Children are placed in the summer education program through their school district Committee on Special Education.
