What is Transition Planning?

Transition planning is a set of coordinated activities, outlined in the Individualized Education Plan (IEP), within the school year that a student becomes 15 years of age.

The activities help students choose and plan for what they would like to do beyond high school. This planning includes:

  • Partnering to develop a road map leading to post high school service planning
  • Utilizing IEP as an ongoing vehicle that reflects and guides the transition
  • Accessing the areas of employment, housing, benefits, medical and leisure services, as well as any other areas believed to be important for the student to become a successful member of the community
  • Preparing the student for the future pathways available after graduation

Jawonio offers a wide variety of individualized transition services provided by an experienced and professional staff responding to the hopes and dreams of your family member or loved one.
